For paroxysmal atrial fibrillation, catheter ablation is already recognized as a first-line treatment; for persistent atrial fibrillation, however, guidelines continue to require an initial trial of antiarrhythmic drugs. The AVANT GUARD study presented today is the first RCT to directly compare pulse-field ablation—a non-thermal, cardiomyocyte-selective ablation modality—with antiarrhythmic drugs in patients with persistent atrial fibrillation, thereby raising the question of whether the current guideline recommendation should be reevaluated.
