Generally, three therapeutically relevant alterations are distinguished: HER2 protein overexpression, HER2 gene amplification, and activating HER2 gene mutations. The diagnostic procedure depends on the specific alteration being detected. Since clinically significant benefits have been demonstrated in various tumor types, accurate determination of HER2 status is particularly important for HER2-overexpressing tumors. For this purpose, tumors are classified as IHC 0 to IHC 3+ according to the more inclusive scoring criteria validated for gastric carcinomas.
