Despite clear guidelines, guideline-based medical therapy for HFrEF is still insufficiently applied in practice, as low blood pressure is often misinterpreted as an obstacle, leading to unnecessary reduction or discontinuation of therapy. Today's position paper therefore presents clinical strategies that enable guideline-based medical therapy despite low blood pressure.
