Prof. Dr. Dirk Arnold, Director of the Asklepios Tumor Center Hamburg and the Department of Oncology, Hematology, Palliative Medicine, Rheumatology, and Pulmonology at the Asklepios Clinic Altona, uses a specific patient case in this video to explain the current evidence-based first-line therapy for unresectable metastatic colorectal cancer with mismatch repair deficiency or high-grade microsatellite instability.
The focus is on comparing PD-1 monotherapy with combination therapy involving PD-1 and CTLA-4 blockade. The key study data for both strategies will be discussed—the KEYNOTE-177 study on pembrolizumab, including 5-year long-term data, as well as the CheckMate 8HW study on nivolumab plus ipilimumab, including current ESMO data from the direct first-line comparison. In addition, factors for clinical decision-making in the tumor board—tumor burden, symptom burden, organ involvement, general condition, and tolerance to toxicity—as well as the role of chemotherapy and the interpretation of early radiological changes during immunotherapy will be discussed.
